Framework establishes physician responsibilities amid clinical AI adoption
The American Medical Association has released a framework defining core physician responsibilities as artificial intelligence and digital health technologies reshape clinical practice. Developed in collaboration with the Digital Medicine Society, the framework emerged from discussions among leaders across medicine, health systems, government, technology companies, and patient advocacy organizations.
The initiative addresses a pressing question: as AI systems take on diagnostic support, administrative tasks, and clinical decision assistance, what remains uniquely and necessarily the physician's domain?
Five enduring responsibilities
The framework identifies five fundamental duties that will persist regardless of technological advancement:
Preserve trust through human connection. Physicians must ensure empathy, communication, and shared decision-making remain central to care, even as digital tools mediate more patient interactions.
Demonstrate and promote clinical judgment. The framework calls for physicians to apply clinical expertise, judgment, and accountability where they create the greatest value for patients—a recognition that not all clinical decisions benefit equally from automation.
Lead the evolution of medical practice. Rather than passively adapting to technology-enabled care models, physicians should actively shape how these models develop, ensuring they improve quality, continuity, and access while preserving high-value physician contributions.
Steward the responsible use of technology. Physicians bear responsibility for guiding the safe, effective, and equitable integration of digital health and AI into clinical workflows.
Advance the profession. This includes preparing future physicians while preserving clinical expertise and ethical decision-making capabilities that could atrophy if over-delegated to automated systems.
Why it matters
As health systems rapidly deploy AI for clinical documentation, diagnostic support, and patient triage, the medical profession faces a defining moment. Without clear articulation of what physicians must continue to own—and what they can appropriately delegate—there's risk that economic pressures will drive technology adoption in ways that undermine rather than enhance care quality. This framework provides a reference point for evaluating whether specific AI implementations strengthen or weaken the physician-patient relationship.
Shared roadmap for implementation
Alongside the framework, the organizations released a roadmap acknowledging that physicians cannot navigate this transition alone. The document outlines how patients, clinical professionals, technology developers, investors, policymakers, and payers must collaborate to create conditions where technology strengthens rather than fragments medical practice.
The roadmap describes a progression from near-term alignment of priorities and capabilities through implementation of new care models toward a future where patients can access timely, clinically appropriate care delivered by empowered physicians supported by trustworthy technology.
Both documents are positioned as starting points rather than prescriptive implementations. The AMA indicates the next phase will require translating these principles into practice through work on evolving roles, physician education and competencies, new care model design and evaluation, and the policy and payment structures needed to support them.
